Overview
This information is for Providers who wish to treat individuals with a substance use disorder that are under the age of 21 using any of the following Early Periodic Screening, Diagnostic and Treatment Services (EPSDT) that are part of the new Medicaid State Plan Services for youth. This would include the following services:
- Crisis Intervention
- Other Licensed Practitioner
- Community Psychiatric Supports and Treatment
- Psychosocial Rehabilitation
- Family Peer Support
- Youth Peer Support and Training
Application
All providers are required to go through the inter-agency designation process and obtain an OASAS operating certificate. If you have not already applied to be designated to provide these services through the inter-agency designation portal, you should begin by visiting the portal and following the instructions to apply.
All other providers, please fill out the attestation Form along with the following abbreviated Certification Application using these instructions. Please submit both forms to [email protected] along with a copy of your designation letter from the Interagency Designation Committee.
For providers currently certified by OASAS and/or licensed by the Office of Mental Health (OMH), fill out this Attestation Form and submit it to [email protected] along with a copy of your designation letter from the Interagency Designation Committee.
