N-propionitrile brorphine has been identified in Upstate New York. This substance belongs to an emerging class of synthetic, lab-made chemicals known as 'orphines' and is not commonly found in the drug supply. Recently, it has been discovered as an unexpected contaminant mixed into fentanyl and medetomidine. Individuals who knowingly intended to take fentanyl or medetomidine were unaware that this other substance was present in their supply.
The New York State Department of Health issued a Public Health Alert on N-propionitrile brorphine on July 1, 2026.
Concerns
- Unknown and Potentially Extreme Strength: The exact potency of N-propionitrile brorphine is not yet known. However, it is structurally similar to another drug in the same family (cyclorphine), which is estimated to be more potent than fentanyl.
- Prolonged Sedation: When N-propionitrile brorphine is mixed with other sedating substances, it can cause heavy sedation for an extended period, making close monitoring and airway management vital.
Safety Strategies
If you or someone you know uses substances, you can follow these health and safety strategies to prevent an overdose:
- Utilize Drug Checking Programs: Encourage individuals to use drug-checking programs to test their substances before use.
- Avoid Using Alone: Do not use alone. If someone is going to use alone, develop a plan so the individual can request and receive assistance if needed.
- Always Administer Naloxone (Narcan): Naloxone should always be administered in any suspected drug overdose, and specifically if the person is not breathing. New York State residents can order free naloxone.
- Provide Rescue Breathing and Airway Support: Because of the potential for prolonged sedation, it is important to ensure the person has an open airway. Administer rescue breathing as needed, place the individual in the rescue position (see below), and continue to closely monitor them after naloxone has been given.
